She is survived by: her mother Marta Afonso; her siblings, Rodney Afonso, Carol Kvidahl, Steven Afonso (Betsy) and Lisa Fuzesi (Michael); her nieces and nephews, Melissa Kvidahl Reilly (Jared), Michael Kvidahl (Melanie), Katelyn, Marisa Afonso, Andrew, Matthew and Christopher Fuzesi; her great-nephew Warren; and her great-niece Violet Kvidahl. She is also survived by many extended family members and friends.

In lieu of flowers, the family requests donations be made in Jacqueline's memory to the American Lung Association or the American Heart Association.
