He was predeceased by: his wife Jeanne; his parents, Edward Zujkowski and Felice Zujkowski; and his brothers, Edward and Joseph.

He is survived by: his children, Alan E. Patrick Zujkowski and Ashley A. Botts (Derek); and his grandchildren, Layla and Alysha Botts.
