He was predeceased by: his parents, Henry Kubacki and Ruth Kubacki. He is survived by: his wife Annette Kubacki; his children, Erika Rose Kubacki, Satin Marie Van Cleef and Henry Kubacki; and his grandchildren, Noah Kubacki, Vienna and Victoria Van Cleef.
