He was predeceased by: his parents, Florence Ciszak and Stanley Ciszak. He is survived by: his sons, David Ciszak of Bayville and Kevin Ciszak (Jodi Ciszak) of Wall; his brother Paul Ciszak; and his granddaughter Emilia Ciszak (Joanne Ciszak).
