He was predeceased by: his mother Margaret Smith. He is survived by: his father Frederick Smith; his wife Catherine; his children, Amanda K. Yeager, Heather A. Grainger and Damien J. Smith; and his siblings, Tom, Ricky and Michelle. He is also survived by grandchildren.
In lieu of flowers, memorial contributions can be made to the American Heart Association or American Diabetes Association.
