He is survived by: his brother-in-law Michael; his children, Rosemary Azzaro (Frank Agostino), John Azzaro (Kathy) and Joann Kozlowski (Stephen); his grandchildren, Meghan Hawkins (Channing), Shannon Azzaro, Krysten Charzewski (Corey) and Stephen Joseph Kozlowski; his great grandchildren, Liam John Hawkins and Alexander James Charzewski; his sisters-in-law, Helen and Rose; Ben. He is also survived by nieces, nephews, colleagues, and dear friends.
