He is survived by: his wife Sandra Perkins; his sons, John Perkins Jr., Jeff Perkins (Heidi Hubert) and Brett Perkins (Susanne Perkins); and his grandchildren, Scott Perkins, Michael Perkins (Annie Apple), Ryan Perkins, Billy Huff (Katherine Huff), Laurel Perkins, Madelyn Perkins and Courtney Perkins.
The family asks that in lieu of flowers that any donations in his memory be made to the Ronald McDonald House, to help families when they may need it most.
