She was predeceased by: her parents, Oscar A. Bazan Alfaro and Berta Isabel Renteros of Bazan; and her sister Bertita. She is survived by: her husband Joseph J. Lewkowicz; her daughters, Nichole Elizabeth Matos (Anthony Joseph Matos), Melanie Anne and Jeanne Isabel; her grandchildren, Gabriel Joseph, Celia Isabell, Wes Charles, Lorenzo Joseph and Luka James; her brothers, Oscar A. Bazan Jr. (Rosa) and Fredrico Jesus Bazan; and her cousin Rosario "Charo" Vasquez. She is also survived by many other cousins, nieces, nephews, and dear friends.
