He was predeceased by: his cousin Stephanie Campo. He is survived by: his fiancée Laura Zienowicz; his fiancée's daughter Hannah; his parents, Bill Hayes and Carol Hayes; and his grandmother Catherine Lynn. He is also survived by numerous aunts, uncles, cousins, friends.

In lieu of flowers, donations may be made in Andrew's name to the Roberto-Venn School of Luthiery, 102 Grand Avenue, Phoenix, Arizona 85007 or Greyhound Angels, 2688 South Black Horse Pike, Williamstown, NJ 08094.
