She was predeceased by: her parents, Frank Kondelka and Margaret Kondelka; her husband John D. O'Hare (Jack); and her brothers, Frank and Al Kondelka.
She is survived by: her grandchildren, Michael (Ryan Mangan), Jennifer Mangan, Matthew Shrauger, Niall O'Hare, Quinn O'Hare, Timothy O'Hare and Noah O'Hare; and her great grandchildren, Ella Mangan, Kira O'Hare and Kyla O'Hare.
