She was predeceased by: her parents, Frank Leonard and Mary Leonard; and her siblings, Eleanor Quinn, Judy Badman, Frank and John Leonard.
She is survived by: her husband Daniel; her children, Erin (Kris) and Dan Bergen (Nadya); her grandsons, Colton, Dylan, Jackson and Ronan; and her sister Kathleen Walls. She is also survived by many nieces, nephews, cousins, and friends.
In lieu of flowers, the family requests that donations be made to St. Jude, a cause close to Phyllis' heart.
