He was predeceased by: his wife Joan A.; his daughter Donna M. Van Kleeck; his brother Thomas Polis; and his grandchildren, Michael Polis, Jaclyn Bayer, Justin Van Kleeck, Kyle Polis, Jason Van Kleeck, Joanna Keegan and Michael Keegan.
He is survived by: his children, Louis A. Polis (Lynn), Jay C. Polis and Anne Marie Keegan (Thomas); his brothers, Frank Polis, Vincent "Jimmy" Polis and Michael Polis; and his great grandchildren, Dylan, Thea, Carter and Zoe.
