He was predeceased by: his parents, Charles Mazzeo and Isabel Mazzeo. He is survived by: his wife Caryn Mazzeo (Srebnick); his children, Cheryl (Axel Jensen), Carissa Jachcinski (Michael) and Charles (Olivia Kwiatkowski); his grandchildren, Sloane, Riley and Axel; his mother-in-law Sandra Srebnick; and his niece Sarah.

In lieu of flowers, the family asks that donations be made to the St. Jude Children Hospital in memory of Charlie.
