He was predeceased by: his parents, Matilda Ortiz and Thomas Ortiz. He is survived by: his wife Jeanne Ortiz; his children, Annette Vega, Denise Concepcion (Arthur), Jessica Berlinski (Keith), Samantha Ortiz, Thomas Ortiz III and Jonathan Ortiz; and his siblings, Nelson Ortiz, Rosemary Ortiz, Yvette Delgado and Zoraida Ortiz. He is also survived by 9 grandchildren and 3 great grandchildren.
