He was predeceased by : his parents, Stanislaw Kosakowski and Adela Kosakowski (Grodzki); and his siblings, Eugene, Walter, Louis Kosakowski, Stella Kosakowski, Jennie Dubil, Anna Rodziewicz, Josephine Mikolai, Irene Wysocki and Mary Faczak. He is also survived by his nieces and nephews.

In lieu of flowers, donations can be made in John's name to the St. Stanislaus Kostka School Endowment Fund.
