He was predeceased by: his mother Kay. He is survived by: his children, Madison, Marlee and Michael Blaze; his close friend George; his brothers, Matt (Denise) and Andrew; and his nephews, Grant, Chase (Cole), Louis Nickolas (Daria) and Evan. He is also survived by cousins, second cousins and very close friends whom he considered siblings.
