He was predeceased by: his parents, Edwin Slack and Ethelda Slack (Howell). He is survived by: his wife Sandy; his sons, Brian (Nancy) and Brent (Chrissy); and his grandchildren, Ashley, Noah, Hayley and Thomas. He is also survived by many nieces and nephews.
