He was predeceased by: his parents, John Smith and Margaret Smith. He is survived by: his wife Annette Smith (Butrico); his daughter Amanda Hackett (David); his granddaughter Emilia; and his siblings, Michael Smith (Maryann), Jack Smith, Tom Smith (Carol), Ed Smith and Peggy Schaeffer. He is also survived by many nieces, nephews and great nieces and nephews.
