She was predeceased by: her father Francis Kosloski Sr.. She is survived by: her daughters, Valerie and Rachael Wade; her mother Eileen Taylor-Levine; her husband Larry; her stepmother Gerri Kosloski; and her brothers, Francis Kosloski Jr., Jason Kosloski (Danielle) and Sean Kosloski (Esteban Medina). She is also survived by her nieces and nephews.
