He was predeceased by: his wife Isabelle; his daughter Kathleen Chaplick; his sister Barbara Bradbury; his parents, Charles and Mary; and his grandson Eric Foreman.
He is survived by: his children, Charlene Forman, Charles John and Jenifer Wintemberg; and his siblings, Wanda Rotan, Lewis Chaplick, Lucille Phillpis and Charles J. Chaplick, Jr.. He is also survived by 10 grandchildren, 11 great grandchildren and many nieces and nephews.
