He was predeceased by: his father Wayne; his grandparents, Donald and Harriet Simmons; and his aunts, Barbara Simmons and Beverly Davis.
He is survived by: his mother Cynthia Simmons; his children, Derek Simmons and Sydney Simmons; his uncle Donald Simmons, Jr. (Brenda); and his cousins, Donald Simmons III, Ashley Fasano (Michael), Stacey Carr (Michael) and Brooke Carroll (Thomas). He is also survived by many extended family members and friends.
In lieu of flowers, donations in Tyler's memory for the benefit of his children's future would be greatly appreciated. Donation envelopes will be available at the funeral home.
