He is survived by: his wife Eva; his children, Renee Martinez, Carla Martinez, Ann Marie Chavez (Robert), Denee Golliheair (Shannon) and Derek Crnkovich (Stephanie); his grandchildren, Taylor, Bailee, Olivia, Jeremiah, Gavin, Sophia, Dalton, Dylon, Logan and Daxton; his great grandchildren, Kasen, Callen, Jose, Cash, Memphis and Meadow; and his siblings, Evangeline Martinez, Monico Martinez (Kathy), Ramon Martinez (Betty), Joe Mercy Martinez (Anna), Irene Garcia (Donald) and Bernadette Martinez. He is also survived by numerous loving nieces and nephews.
In lieu of flowers, the family respectfully requests that donations be made to the Leukemia & Lymphoma Society (LLS), also known as Blood Cancer United, in Jerry's memory.
