He was predeceased by: his parents, Harley Ballard and Alice Ballard; his former wife Carolyn Lee Ballard; his daughter Konnie Eigsti; his grandson Graham Arthur Jurva; and his siblings, Jerry, Harley, Tooks Ballard and Mittie Smart.
He is survived by: his wife Mary Lou Ballard; his daughters, Sandy J. Ballard (Rachael), Katie Townsend (Ross), Krista Jurva (Nathan) and Kari Putman (Jon); his son-in-law Mike Eigsti; his grandchildren, Austen Eigsti, Brianne, Zack Putman (Teagon), Bryce Kershaw (Gabe), Aubry Putman, Cydnee Eigsti, Jack Jurva, Payton Putman, Morgan Townsend, Kelly Jurva, Luke Jurva and Reid Townsend; his great grandchild Hope Eigsti; and his sister Kathleen Hood. He is also survived by numerous nieces and nephews.
