He was predeceased by: his parents, Stanley Ptak and Ida Ptak. He is survived by: his siblings, Steven Ptak, Carol Ornowski (David), Timothy Ptak, Barbara Fedchak (Bill) and David Ptak (Judy). He is also survived by 10 nieces and nephews; 6 great-nieces and great-nephews; and many cousins and friends.
