He was predeceased by: his parents, Martin Kazala and Florence Stark Kazala; and his brothers, Larry, Richard and George "Pudgie" Kazala.
He is survived by: his wife Michele; his daughter Nancy (Carmen Corvigno); his grandchildren, Alyssa, Chrissy and Kenny; and his siblings, Tim Kazala (Nancy) and LeeAnn Kazala. He is also survived by several nieces, nephews, cousins and friends.
In lieu of flowers, memorial contributions may be made in his memory to St. Mary's Church or St. Casimir's Cemetery Fund.
