He was predeceased by: his parents, Charles S. Shaw and Lola M. Shaw (Storie); and his brother Robert Shaw. He is survived by: his wife Leona M. "Toots" Kirkby; his children, Lori (Claude "Spence" Ritchie), Lisa Maloy (John), Penni Shaw-Pierce, Andrew Shaw (Brooke) and Richard Shaw; his sister Cynda McKeever (John); and his grandchildren, John Shaw (Alyssa), Kara Rockwood (Ben), Laura, Spencer, Jackson Ritchie, Charlie, Connor Shaw, Andrew, Christopher Pierce, Adam and Leah Shaw.

In lieu of flowers, donations in memory of Jack may be made to the Indian River Ambulance Service.
