She was predeceased by: her parents, Carl Perkowski and Regina Perkowski (Maley); and her grandparents, Teofil, Amelia Perkowski (Kloss), John and Helena Maley (Lippold).

She is survived by: her daughters, Nicolle Fortes and Courtney Perkowski; her grandson Drew Fortes-Crimes; and her brother Joseph Perkowski.
