He was predeceased by: his mother Anne Czapla. He is survived by: his wife Maria (Fabila); his children, Jamie Martin (Brandon), Joshua (Ruthie), Jessica and Jacqueline Czapla; his father John S. Czapla; his grandchildren, Wyatt, Everly, Jillian, Damien and Harlow; and his siblings, Howard (Christine), John and Thomas Czapla (Linda). He is also survived by nieces, nephews and cousins.
