Dake Forward of Franklinville; his children, Jenna Forward of Springfield, MA. and Jason Forward of Springfield, MA.; his sisters, Carol Forward and Tammy Woodward of Franklinville; and his aunts, Doris and Jackie. He is also survived by many nieces and nephews.

A memorial service will be held on Saturday, May 11th 2024 at 1:00 PM at the First Baptist Church (27 S Main St, Franklinville, NY 14737). A reception will be held on Saturday, May 11th 2024 at 3:00 PM at the Ischua Valley Country Club (8093 NY-16, Franklinville, NY 14737).
