He is survived by: his wife Tammy Jo Urik (Kilgore); his sons, Mike Urik (Heidi Tady) and Nick Urik (Sara); his grandchildren, Savanna Urik, Landon Beckett, Dylan Urik, Kinsley and Aislyn Tady; and his siblings, Jeff Urik of Allegheny Twp, Janet Boehm of Sarver and Joan Rutkowski (Rex) of Freeport.
