He was predeceased by: his wife Anne B. (McHenry); his son Andrew Czapla (Maria); and his siblings, Patricia, Victor, Louie and Helena.

He is survived by: his children, Howard (Christine), John and Thomas (Linda); and his grandchildren, Elizabeth (Jeff), Nicholas, Robin, Jamie (Brandon), Josh (Ruthie), Jessica and Jacqueline. He is also survived by nieces and nephews.

If desired, donations in John's memory may be made to the Assumption Church Preservation Fund.
