He was predeceased by: his parents, Anthony and Jeanette. He is survived by: his wife Elizabeth; his children, Sara Murray (Ryan), Joshua, Jenna O'Connell (Andrew) and Jack; his siblings, Marlene, Eileen, Kathy Meyer (Tim) and Daniel Caccamise (Geanine); his mother-in-law Claire Boutillier; and his siblings-in-law, Rob, David Boutillier (Ellen), Sue Vinton (Mike), Cathe Muzytchuk (Don) and Barb Boutillier. He is also survived by many nieces, nephews, cousins, and close friends.

In lieu of flowers, donations may be made to The Patricia Allen Fund benefiting the John R. Oishei Children's Hospital of Buffalo.
