She is survived by: her spouse Thomas J. Usiak; her children, Thomas Usiak (Christine Peterson), Jonathan Usiak (Caroline Rivard RN) and Jennifer; her grandchildren, Ryan, Samantha, Allison Usiak, Jacob and Anna Altshuler; and her stepgrandchildren, Abigail and Ian Levick.
