He was predeceased by: his wife Sarah Jeacock. He is survived by: his sons, Dr Thomas L., David A. (Grace) and John J. (Alice); and his grandchildren, Richard W. (Andrea), Lyman B. (Joanne), Patrick J, Grace L and Margaret A. He is also survived by three great-grandchildren.
In lieu of flowers, Memorial Contributions may be made to the University of Buffalo School of Medicine or the Oshei Children's Hospital.
