He was predeceased by: his father Earl Cook; and his brother Steven Cook. He is survived by: his mother Doris Cook (Gallien) of Chateaugay; his grandson Lincoln Scott Cook; his son Zachary; his daughter-in-law Courtney Cook of Chateaugay; his grandson Lincoln Cook; his siblings, Eric Cook (Francine) of Chateaugay, David Cook of Bangor and Tammy Cook of Cambridge, NY; and his siblings' children, Carissa Cook, Tarl and Easton Cook.
