He was predeceased by: his parents, William C. Koubek and Loretta M. Stearns Koubek; and his wife Annette M. Higby. He is survived by: his daughters, Pamela L. Koubek Cheektowaga (Melvin Gonzalez) and Michelle R. Headley (Tim) of Nampa, Idaho; his grandchildren, Jacob, Bailey, Aliana, Matthew, Michael and Nicholas; and his sister Gail O'Rourke Howey (Gary).
