He was predeceased by: his parents, Edward L. Siudak and Alfreda Siudak; and his sibling's children, Sheila and Thomas Czosek. He is survived by: his daughters, Maria and Julie; his sister Diane Raczka (Robert); and his nephew Michael. He is also survived by six grandchildren.
