She is survived by: her parents, Nick Maciag and Katie Maciag; her grandparents, Dennis, Camille Maciag, Herb Klein and Maureen Murphy; her great-grandparents, Roland and Lottie Pikuzinski; her aunts and uncles, Rob, Gabby Klein, Berg and Kim Rozanski; and her cousins, Alyssa, Izzy, Kole Klein and Maddie Rozanski.
The family asks that in lieu of flowers that donations be made in Evelyn's memory to The Infant Brain Tumor Fund at Dana-Farber Cancer Institute.
