He was predeceased by: his wife Dorothy E. Koszuta; his parents, Anthony Koszuta and Ida Koszuta; his son David Koszuta; and his siblings, Joseph (late Dorothy), Raymond (late Freda), Florence Skotarczyk (late Joseph), Mary Berkowski (late Henry) and Agnes Berkowski (late Henry).
He is survived by: his children, Shirley Root (Lee) and Gary (Celeste); his grandchildren, Rachel and John; and his great grandchildren, Gavin and Audrey.
