With heavy hearts, we announce the death of Sally A. Modica of Falconer, New York, born in Jamestown, New York, who passed away on June 6, 2021 at the age of 76. Leave a sympathy message to the family on the memorial page of Sally A. Modica to pay them a last tribute.

She was predeceased by : her parents, Ruth Nelson Nord Johnson and Ernest LeRoy Nord; her friend Sandy Wallin; her stepfather John "Jack" Johnson; her brothers, David and Nelson Nord; and her stepsiblings, John Johnson, Gaynelle Dickerson and Robert Johnson.

She is survived by : her children, Jennifer Puleo (Jeffrey) of Lakewood, NY and Jason Modica (Kelly Bostwick) of Falconer, NY; her grandchildren, Adam Puleo (Samantha), Markus Puleo, Sarah Modica and Andrew Modica; her great grandchildren, Avery and Abel Puleo; and her brother Stuart Nord of Jamestown. She is also survived by several nieces, nephews and cousins.

In lieu of flowers, memorials may be made to Chautauqua Hospice and Palliative Care, 20 West Fairmount Ave. Lakewood, NY 14701.
