He was predeceased by : his parents, John S. and Lottie M. Luzak Sr. (Mietlicki); and his siblings, John S. Luzak Jr., Gerald Luzak, Robert Luzak, Richard Luzak, Larry Luzak, Linda J. Davis and Diane Luzak.

He is survived by : his wife Barbara A. Luzak (Dominski); his sons, Dennis Luzak Jr. (Bridget) and Robert Luzak; and his brother Ronald Luzak. He is also survived by five grandchildren; one great-grandchild.
