She was predeceased by: her grandparents, Shirley Jane Cook and Billie James Cook; her aunt Leslie Lee Cook; her uncle William James Cook; her cousins, Linda Ann Cook and Philip Andrew Cook; and her niece Anna Marie Christy.

She is survived by: her parents, Darlene Warren (Cook) of Westfield, NY and David Mastalinski of Buffalo, NY.; her daughters, Jasmine Cook of Jamestown, NY and WinterRose Carr of Busti, NY; her stepfather Gary Warren II of Westfield, NY; her sisters, Tabitha Dellahoy (Dave Kilian) of Ashville, Destinie Christy (Randy) of Bemus Point, Lacey Dellahoy of Westfield, Sarah Ramey (John) of Bemus Point, Stephanie Dellahoy of Jamestown and Samantha Colvin of Jamestown; her stepbrothers, Allan Warren (Carley) of Mayville, Nelson Warren of Northeast, PA and Gary Warren III of Minorsville, PA; and her stepfather Michael Dellahoy (Cindy) of Ellery Center. She is also survived by several nieces, nephews, aunts and uncles.
