He was predeceased by: his parents, Kenneth E. Dack, Sr. and Marion "Betty" Dack (Kennedy); his brothers, William, Richard and Donald Dack; and his brother-in-law Dave Iamele.
He is survived by: his children, Michael Dack (Lisa) of Waterford, NY, Michele Wilson (Dave) and Kelly (Dack); his grandchildren, Jennifer Lankford (Chad), Christopher Swienton (Julianne), Julie Parnell, Paul Dack (Katie), Stephanie LeClaire (Geoffrey), Michael Swienton (Brittany) and Samantha Cardinal (Jake); his great grandchildren, Jude, Grant, Arabel Lankford, Parker, Jackson Parnell, Abigail, Christopher Swienton, Ariana LeClaire, Everly and Madeline Cardinal; his sister Linda Iamele; and his wife Judy Applegate. He is also survived by several nieces, nephews, cousins, neighbors, and special friends.
