He was predeceased by: his parents, John Cook and Anna Barto Cook; and his brothers, Ronnie Cook, John Cook, Jr. and Raymond Cook.
He is survived by: his brother-in-law Harold Weise; his wife Alice Maxine Rogerson Cook; his daughters, Rhonda Beardsley (Bryan) of North Carolina, Susan Young (Kevin Allen) of Lakewood and Janice Munson (Joseph Brophy) of Canastota, NY; his grandchildren, Matthew Beardsley (Philip Malcolm) of Florida, Justin Beardsley (Elizabeth) of North Carolina, Nathan Beardsley of Florida, Jessica Menezes (Eric) of Wisconsin, Lance Milks (Melissa) of Ohio, Christopher Milks of Canatota and Richard Allen (Katie) of Frewsburg; his great grandchildren, Jada, Lyrik, Leah, Charlotte Milks, Hudson Miller, Anna, Anthony Beardsley, Tristan, Benjamin, Colette and Elina Menezes; his brother Thomas Cook (Connie) of Altoona, PA; and his sister-in-law Elaine Cook of Pinecroft, PA.
