He is survived by: his wife Betty Hockenberry Crooks; his daughters, Debbie Smith of Jamestown and Amy Rhodes (Jeffery) of Jamestown; his siblings, Linda Pascarella, Joy Pascarella of Bradford, PA and Jack Crooks of Las Vegas, NV; his grandchildren, Jessica Crooks, Lindsey Bloomberg of Jamestown and Zachary Rhodes of Jamestown; and his great grandchildren, Mia Heggerty, Cheyenne Smith, Zade Van Hook, Tessa Wells and Allanah Bloomberg.
