Hoffman; his siblings, Jerri Telford (Chuck) of Jamestown, NY and Chuck Bull (Jo) of Apollo Beach, FL; his mother-in-law Madeline Hoffman of Newton Falls, OH; his brother Mark; and his sister-in-law Aprile Hoffman of Poland, OH. He is also survived by several nieces and nephews.

Memorials may be made to the American Cancer Society, Jamestown Lions Club, Mile High Freedom Band of Denver or the Gay Men's Chorus of South Florida, Fort Lauderdale, FL.
