She is survived by: her husband Jake Slack; her children, Anthony Slack (Stephenie), Michelle Cook (Mike), Annette Mott, David Slack (Holly), Evan, Collin, Victoria, Landon, Holden Slack, Emma, Naomi Cook, Derek, Natalie Mott, Dan and Evelyn Slack; and her siblings, Nina Perry (Don), Mike Quail (Missy), Mitch Quail (Eileen), Tina Sturdevant (Bill), Andy Quail, Andrea Hill (Mike), Jay Quail (Robin), Yvonne Canniff (Shawn) and Suzan Tedder (Chad). She is also survived by in-laws, several nieces and nephews, and numerous extended family.
