He was predeceased by: his parents, Bernard Kantak and Frances Kantak (Carlson); and his brother Bernard J. Kantak, Jr. (Bob). He is survived by: his wife Cheryl L. Kantak (Heyman); his daughters, Cindy and Deb; and his brother-in-law Scott Heyman (Linda). He is also survived by several nieces, nephews, and cousins.
