He was predeceased by: his parents, Wayne Gage and Olive Gage; and his brother Gary Gage. He is survived by: his wife Carol; his children, Craig Gage, Jr. (Susan) and Kelli Ward (Douglas); his grandchildren, Gavin, Addison, Sophia, Owen and Nathan; and his siblings, Kathy Stott (Robert), Pamela Scalfani and Jeffrey Gage (Kim). He is also survived by several nieces, nephews and cousins.

In lieu of flowers, memorials may be made to Roswell Park Cancer Institute.
