He was predeceased by: his parents, Kenneth Lewis and Betty Lewis (Olds); his girlfriend Linda Urbach; and his son Casey. He is survived by: his child Shelly Taylor (Jeremy) of Coleman, MI; his stepchildren, Sharon Izzy (Michael) of Hamburg, NY and Douglas Murphy (Jamie) of Portland; and his brother Greg Lewis (Sue) of Orlando, FL. He is also survived by seven grandchildren, and four great-grandchildren.
